Ways to Wellness is on Target

We are pleased to report that Ways to Wellness is on target with referral levels. Since April 2015, we have been working with 416 patients with long term conditions, referred from the 17 GP practices in the west of Newcastle.

Now that we have been working with our first patients for just over six months, we will soon be able to measure improvements in their health and well-being.

Impact and Patient Benefit

Although we are still early in Ways to Wellness’ service delivery, we are starting to look at the data we have collected thus far. Combining this information with the experience and insight of those providing the service (Link Workers and provider managers), we aim to build a better understanding of what is working well. Our goal is to identify and build on the factors that optimise patients’ ability to improve their health and well-being through their involvement in the Ways to Wellness service.

Ways to Wellness continues to attract interest in its pioneering approach. NHS Clinical Commissioners (NHSCC) launched a new publication in September 2015, Transforming healthcare in England’s core cities, highlighting Ways to Wellness as an innovative service that benefits the local population.

Building Awareness of the Ways to Wellness Service

Big Society Capital is producing a film of Ways to Wellness. We will post a link to this video on our website once the film production is complete. The film captures a numbers of perspectives of people involved in Ways to Wellness, including patients, Link Workers, investors and commissioners. It also highlights the aspects of Ways to Wellness that are innovative and pioneering – social impact funding and large scale social prescribing.
